Sojourns Community Health Clinic

is an ambitious attempt to offer a solution to the diverse
problems of the current health care system.
Who We Serve

Our practitioner team meets every day to discuss cases
and integrate care. Semi-monthly full-staff meetings
ensure continued involvement of the entire team in Sojourns’
evolution.

us to offer the following modalities, assessment
