If you want to use health insurance for the visit, please contact your insurance provider to make sure your individual policy covers both the practitioner (using the NPI Numbers below) and the service, for example primary care, consultation, chiropractic, etc. In addition, please find out if your coverage has a deductible amount to be met or a co-pay amount for the service. We advise you to get the name of the representative and reference number as well as the date you called your insurance.
